Appointments
MDRI accepts most major insurances in addition to workers’ compensation and Medicare. At MDRI our goal is to maximize your insurance coverage and assist with providing the highest level of care your benefits will allow.
In order for our office to submit your medical claim for you, it is necessary that you provide our office with your medical insurance information. Your insurance cards are required even if you have workers’ compensation or automobile insurance coverage. If you have medical insurance that requires a referral provided by your family doctor or authorization, please be sure to bring the referral with you to your appointment. This information is essential for each and every visit.
If your insurance coverage has a copayment and/or coinsurance, you will be required to pay this at the time of service. In the event insurance cards and referrals are not provided and/or copayments and/or coinsurances are not paid, the provider may reschedule your appointment. Our office accepts cash or personal check.
It is necessary that you bring with you any x-rays or MRI films recently taken related to your current problem, as well as your medical records from your referring physician. If you need assistance with this, please contact our office in advance.
You will find forms needed for your initial appointment on our website, in addition to directions to our facilities. We ask that you arrive 15 minutes earlier to complete addition paperwork. You should anticipate your visit to our facility to be approximately 45-60 minutes. Routine follow-up appointments will require 15 to 30 minutes of your time. We are looking forward to meeting you and doing our best to serve your needs.
Prescriptions
Our offices specialize in physical medicine and rehabilitation focusing on education and lifestyle change. Your treatment plan may include prescription medication. We take prescribing medication very seriously and expect you to do the same. Medications have documented side effects and must be taken exactly as prescribed. Patients who do not take medications as prescribed may be discharge from our practice due to non-compliance. Medications are closely controlled by local, state, and federal governments, as well as by our offices. These medications only block the symptoms and do not correct the underlying problems.
Patients of MDRI are expected to be responsible for their prescriptions and obtaining refills of those prescriptions. All MDRI facilities require a minimum of 24 hours to process prescriptions. Refill requests are processed based on the order they are received. MDRI providers and staff will manage your request efficiently. Patients are required to provide MDRI providers and/or staff with the following information to handle your request in a timely manner: spelling of first and last name, date of birth, spelling of prescription, dosage, quantity requested, name and phone number of pharmacy, mailing address (if prescription is to be mailed), and patient telephone number. Requests that do not meet this requirement may not be processed in a timely manner. MDRI will notify patients when their request has been complete.
Patients are encouraged to communicate any side effects, questions, concerns or changes in their health history or medications with MDRI providers and staff to ensure patient safety and effective treatment outcomes. If you think you are experiencing side effects related to your medication, or do not feel the medications are being of benefit, please contact MDRI immediately so that we may assist you.
