 NOTICE OF OFFICE CLOSING

Please be advised that effective September 30, 2026, Morganstein De Falcis Rehabilitation Institute (MDRI) will be permanently closing our outpatient practice. Thank you for trusting us with your healthcare needs over the years.

TRANSFER OF CARE:  Because your healthcare requires continuous care, we strongly advise you to establish care with a new healthcare provider immediately.

MEDICAL RECORDS:  Your medical records will remain confidential and secure.  To obtain copies or request a transfer to your new physician, we will require a records release form.

PRESCRIPTIONS:  No prescriptions will be issued after our closing date of September 30, 2026.

APPOINTMENTS:  Appointments will be honored up to Wednesday, September 30, 2026. Any appointment scheduled after that date will be automatically canceled.
